I require a single user MS Access database to help track incidents that occur in a correctional facility. The database must support the following type of queries.

By Case:

• Case status
• Status date
• Involved officers
• Involved inmates

General:

• Number times inmate in incident either as a victim, witness, or accused
• Number of times officer involved in incident either as victim, witness, or accused
• Number of open incidents per investigator

A data entry form is also needed. Listed below is some entity information.

Officer:
ID (4 digit numeric)
Last name
First name
Status (Victim, Witness, Accused, Investigator)

Inmate:
ID (digit alphanumeric)
Last name
First name
Status (Victim, Witness, Accused)

Case
Number (10 digit alpha numeric)
Status (open, Closed, Deferred)
