I need someone to create two web based forms that work with normal mysql database.

1. Form must be printer friendly
2. As we add the clients info: Name, address,zip,phone must be able to choose between the list of clients entered or have option to enter new client.
3. Drop down menu with 20 - 30 options - need to be able to add options later if we needed.
4. Would like to be able to create an invoice from the first form just copy some of the info of the first form to the second and either print or email to the client.

This is a simple and fast form should not take more than a few hours at most.
I have attached a PDF file that is a sample of what the fields will be a couple of small changes but that form is the first one - the invoice form just needs to look like an invoice.

We need to store the forms in the database and have them green when we first enter them in and turn blue when we mark them complete.

Also need a simple user name and password so that each employee has to login.

There are many reasons for this including version incompatibility, restricted permissions, etc... Rent A Coder normally requires that the worker verify in advance that your 3rd party hosting provider will support the components.  If they do not, then they are responsible for any incompatibilities, rather than you. If this contract makes it impossible for a competent person to do this, then do not start this bid request...but instead alert Rent A Coder of an un-arbitratable, illegal bid request.
