Brief Summary:

I need a fairly basic web page created to input data into a MySQL database.  The page will have between 10-20 inputs, all pretty standard.  A few of the bells and whistles this site needs are:
  1. Ajax real-time username checking.  This should return a simple 'This username is available/taken' message.
  2. Basic validation for Email, Address, etc.
  3. Attach file button.  This should provide a simple and intuitive panel for a user to upload a document (*.doc, *.docx, *.pdf, etc) to our server either from their computer or from a web link.  I'm not exactly sure what the best way to handle this would be so hopefully you can provide some insight.  I think it would be something like it renames the filename to a random unused hash and just stores a reference to it in the database.  There should be a size and type limit and some kind of security measures in place.
  4. Attach/upload image similar to the document.  It would be nice if the image could be automatically resized or cropped and a control for the user to choose the cropping region would be great.
  5. A confirmation email should be sent to their email containing both a clickable link and a random code.  The user account should not become valid until they click or enter this code in the system.
The pages should follow XHTML1.0 strict doctype and use javascript and PHP5.  The code should be very clean and well documented.  You are welcome to use javascript libraries, such as jquery, but must make sure the licenses work for a commercial company.  Don't worry about design or css.  Functionality is key.  I was told to expect a poor quality of work through outsourcing, so I will provide extra compensation if my low expectations are exceeded.

No. The worker is responsible for creating their own development and/or qc environment.
Legal: 1) I require complete and fully-functional working program(s) in executable form as well as complete source code of all work done (so that I may modify it in the future).
2) Deliverables must be in ready-to-run condition as follows (depending on the nature of the deliverables):
2a) All other software (including but not limited to any desktop software or software the buyer intends to distribute) must include a software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request (unless specified elsewhere by the Buyer).
3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased.
3b) No part of the deliverable may contain any copyright restricted 3rd party components (including GPL, GNU, Copyleft, etc.) unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the worker's Worker Legal Agreement.
Other Requirements:
  • All deliverables must be uploaded to Rent A Coder before the deadline(s) for this bid request...with no exceptions. If this contract makes it impossible for a competent person to do this, then do not start this bid request...but instead alert Rent A Coder of an un-arbitratable, illegal bid request.
  • Remember that contacting the other party outside of the site (by email, phone, etc.) on all business bid requests <$500 (before the buyer's money is escrowed) is a violation of both the software buyer and worker agreements.
